Hole covers are required for all floor openings and holes greater than two inches by two inches
in size. The covers should be marked with the words HOLE or COVER DO NOT REMOVE. The cover
should be secured for movement. Make sure the cover will support two times the expected
load. Both indoor and outdoor work areas can contain step and fall hazards. Step and fall
hazards are created by objects such as rocks on the normally flat or clean surface. When
stepped on, these objects can cause the ankle to turn and the person to fall forward. When
work tasks require focus above the work area or there are shadows or lighting changes, an
unintended trip and fall hazard can be made much more serious.
Clearing the work area before work begins can avoid serious ankle injuries.

Good barricading installed before the hole is created is an important safety practice
that will help prevent trips and falls.
[Music] Remember when removing a cover - either temporary or permanent - it must be replaced
when the work is completed. Holes deeper than four feet require guardrails. Painful foot,
ankle, and leg injuries from these types of step and fall hazards are possible. Always
barricade and prevent. All work areas should be evaluated for potential trip and fall hazards.
Often, permanent installations will create inherent tripping hazards near walkways and
stairs. Always look where you will be walking first, and then decide a safe course of action.
A moment of awareness can prevent a painful trip and fall injury. Empty pallets and packaging
shrink wrap can create serious tripping hazards. When any tripping hazard is located in a walkway
or natural travel path that is not visible, a serious hazard is created. Stack empty pallets
in a safe location and remove used shrink wrap from the floor. All material cutoffs and
scrap must be cleaned up as the work is performed. Rounds such as conduit and all thread are
especially dangerous as they can roll under the foot when stepped on. A receptacle for
scraps should be located next to the saw.
